Provides day-to-day supervision and living skills instruction for the residents and carries out duties as assigned by the House Manager or Residential Coordinator. This includes providing training and support for residents to help them attain maximum independence and maintenance of appropriate documentation. A Residential Supervisor is required to use independent judgment (or works under the direction of supervisor) when performing job tasks/providing services. As part of Centerstone’s team-oriented philosophy, a Residential Supervisor is expected to actively participate in variety of cross-functional teams and performance improvement initiatives.

ESSENTIAL DUTIES

Assists service recipient in developing an individualized treatment plan, must be person centered, family centered, and service centered. Monitors service recipient progress in treatment and participates in treatment plan reviews as assigned. Updates treatment plans as appropriate. Submits treatment plans at all stages for medical review, if applicable. (clinical/direct service).
Assists service recipients with making linkage with internal and external resources as outlined in treatment plan and in coordination with Integrated Healthcare Services. Maintains appropriate contact with referral sources including providing information on service recipient progress, behavior, and needs (with appropriate releases), and maintaining ongoing consultation when appropriate.
Provides transportation for service recipients as needed.
Provides personal care/physical assistance as needed.
Attends staff meetings, inter-disciplinary staffings, and other internal meetings as assigned. Participates on committees as assigned, including quality assurance functions. Maintains appropriate communication with other employees and work as an effective member of treatment team.
